How do request equipment?

1. Download and fill out the attached Order Form (mv_order_form.pdf) or go to   https://case.edu/utech/sites/case.edu.utech/files/2018-05/mv_order_form.pdf.
2. Fax the form to (216) 368-3408 or email it to mediavision@case.edu. MediaVision will receive the request and reach out regarding availability and to confirm the request.
